The print out of the online application duly recommended by the Joint Director of Health Service cum Member Secretary, District health Society of the respective district along with attested copies of the relevant documents shall be submitted of the Mission Director, NHM, Assam, Saikia Commercial Complex, Srinagar path, Christan Basti, Guwahati-5, Envelopes containing application should be super marked: Application for Diploma in Mental Health/ Pediatric Medicine/ Clinical Anesthesiology/ Radiology/ Orthopedics
